The New Uniontown Health Hub: A Transformative Resource for Fayette County
In September 2024, Adagio Health proudly opened the Uniontown Health Hub at 2 Highland Park Drive in Fayette County. Building on more than 45 years of service in the Laurel Highlands region, the new Health Hub will serve as a comprehensive healthcare resource for the local community.
Adagio Health has long been a trusted provider of inclusive, compassionate care to individuals from diverse backgrounds, including veterans, immigrants, and members of the LGBTQ+ community. The new Uniontown Health Hub expands on this commitment by offering a wide range of services designed to meet the unique needs of the community, including:
- Reproductive health care such as gynecological exams, pap tests, and contraception counseling
- Breast and cervical cancer screenings, with referrals for mammograms and diagnostic services
- Trauma-informed care, particularly for veterans and those with histories of trauma
- Behavioral health services, including screenings and counseling for mental health and substance use disorders
- Preventive services for diabetes, hypertension, cholesterol, and HIV
- Care navigation, connecting patients with essential resources like health insurance, food assistance, and housing support
The Uniontown Health Hub also provides patients who test positive for food insecurity with emergency food bags to go, while ensuring they are then quickly connected to local food banks and other sources of support, reflecting Adagio Health’s commitment to addressing social determinants of health. Many of the patients we serve live at or below the federal poverty level, and by offering holistic, patient-centered care, we aim to support not just individual patients, but their families and communities. The Uniontown Health Hub is positioned to have a transformative impact on Fayette County, improving health outcomes and enhancing the overall quality of life for the residents.
Intelichart Rollout: Improving the Patient Experience
