Founded in 1971, Adagio Health operates medical offices and partners with more than 20 other health care services to provide health care at more than 55 sites in 23 counties of western Pennsylvania.  Adagio Health addresses a wide variety of health concerns, such as obstetrics and gynecology, breast and cervical cancer screening, STI testing and treatment, nutrition counseling, teen pregnancy prevention, HIV prevention, sexuality education for teens, community education, parent education and professional training.  Donors make it possible to help us care for the most vulnerable, especially those who rely on Adagio Health Inc. as their sole health care provider.

Is my gift tax-deductible?
Yes, Adagio Health Inc. is exempt from federal income taxes under Section 501(c)(3) of the Internal Revenue Code. Donations are tax-deductible to the fullest extent allowable under the law.

Can I donate to my local Adagio Health center?
Yes. To donate to a local office online, add the name of the center in the note section of the gift form.

Can I send a donation in the mail?
Yes, we accept check and credit card donations by mail. Please print this PDF form and mail to:

Can I donate by phone?
Yes, you can call us toll-free at 1-800-215-7494 to make a secure donation with one of our representatives.
We are available to take donations during the following hours:
Monday through Friday: 9:00 AM – 4:00 PM EST

Are online donations secure?
Yes. Your security is important to us. We follow industry best practices to ensure your personal and credit card information is kept private and secure.  Adagio Health’s donation process is PCI compliant and we use SSL technology to encrypt personal information. Look for the yellow Norton Secured Seal near the submit button on our forms, which indicates our site passed a daily scan and is verified by Symantec. If you prefer, you can also contact us toll-free at 1-800-215-7494 to make a secure donation with one of our representatives.

I’m a monthly donor. How do I update my billing information?
You can update your billing information by contacting us at 1-800-215-7494. Please be prepared to provide the full name, email address and billing address used when setting up the monthly donation so we can locate your gift.
